Our dedicated billing team ensures every client receives clear communication regarding insurance coverage, out-of-pocket costs, and available financial options.
Comprehensive Billing Support
AFCC offers full billing and claims management services for all current and prospective clients.
Our Billing Manager confirms insurance eligibility, verifies benefits, and manages claims processing to ensure accuracy and timely filing.

Payment Methods
To make the payment process easy, we require all clients to keep a credit or debit card on file when completing intake paperwork through our secure, HIPAA-compliant portal.
After each session, your card will be processed for any applicable:
Session fees
Co-pays or deductibles
Missed appointment or late-cancellation fees
We accept:

Clients using FSA or HSA accounts may request an itemized receipt for reimbursement.
Special payment accommodations can be arranged upon request.
Missed Appointments & Cancellations
We value your time and reserve appointment slots specifically for you.
Please provide at least 24 hours’ notice to cancel or reschedule an appointment.
Appointments canceled with less than 24 hours’ notice—or no-shows—may incur a $30 fee, which cannot be billed to insurance.

AFCCChicago is in-network with many major insurance plans for behavioral health, mental health, and DUI services, including:

If your plan is not listed, services may be available under out-of-network benefits.
We will provide a superbill (itemized treatment receipt) for you to submit to your insurance company for direct reimbursement.
Out-of-Network & Private Pay
If AFCC is not contracted with your insurance plan, you are responsible for the full session cost at the time of service.
Many PPO plans (like BCBS PPO) still reimburse for out-of-network behavioral health services.
We recommend calling your insurance provider to verify coverage before your first appointment.
Please note: Medicare does not reimburse for out-of-network services.
Sliding Scale & Financial Assistance
AFCCChicago proudly offers a Sliding Scale Fee Program to make treatment affordable for all clients, based on gross household income.
Clients may qualify for reduced fees by submitting proof of income (recent pay stubs or tax returns).
Sliding scale and payment plans must be reviewed and approved by management.
Our Patient Care Coordinators are happy to assist you with the application process.
DUI Program Payment Policy
For clients participating in our Illinois-Approved DUI Risk Education and Treatment Programs:
A 20% down payment is required to begin services.
A 5% convenience fee applies to all payment plans.
Payment plans are available in 2, 4, 8, 16, or 20 payments, depending on program length.
$20 late fees apply to missed or delayed payments.
Early payoff rewards are available for clients who complete payments ahead of schedule.
Sliding scale options are available for qualified clients.
